Participant support

The information on this page is a guide to help you understand and navigate UB's systems. Do not take anything here as official UB policy, nor as a commitment from UB or from Dr. Proctor.

Participant support payments are paid to people receiving education or training. Participant support payments are sent by check to your home address.

Paperwork

You will need to complete several forms and upload them to a UB Box drive. (Dr. Proctor will provide you with the link.)

  1. Complete the SUNY RF Academic Support/Participation Stipend. Instructions:
    • Top section:
      • Check the box for participation stipend.
      • Department: Learning and Instruction
      • PI/Contact: Chris Proctor
      • Phone: 716-645-2455
    • Administrative data: Complete this section, but do not add a social security number. You can skip any questions you do not understand (e.g. check distribution code)
    • Academic fellowship data: Skip this section.
    • Participation/stipend data: Complete this section with information provided by Dr. Proctor.
    • Declaration: Sign and date.
  2. If you are a US citizen or a resident alien:
    • Complete form W9.
    • You are not subject to tax witholding
    • If you are being paid over $600, you will receive a 1099 form.
  3. Otherwise (If you are not a US citizen or resident alien):
    • Complete form W8BEN
    • Your payment is subject to 30% tax withholding.
